Your Doctor Program, L.P. has developed an automated quality model that positions the medical home as a care coordination hub within a telecommunications system. This model is wired with telemedicine to enhance access to specialty care via “curbside consults” and live interactive video services for communication within the patient’s care network.
As part of the medical enrollment process we define the process to link the medical home into a local health information exchange.
How do we help patients?
We provide the answer to the questions, "who is a good doctor" and "how do you find one?". Once you are enrolled into a YDP Certified Medical Home, we provide a streamlined method for managing your health information to assure that care is coordinated and your doctors are on the same page. We provide 24 / 7 access to a doctor that has your medical information. We increase access to physicians through email and telemedicine.
How do we help doctors?
People expect their primary care physician to be their point of health care access and an assurer that their care in the system is evidence-based, cost-effective, coordinated, and safe. Since primary care physicians or specialists who serve as the patient’s primary care do not have the systems in place nor are paid for these services, we provide a system and payment to provide these services. We offer practice-based continuing medical education for physicians and their office staff. We also offer a service to assist physicians to incorporate telemedicine and other e-health initiatives into their practice.
How do we help hospitals?
Getting physicians involved in evidence-based care, quality improvement and assuring that all of the doctors are on the same page for patient care is a challenge. Our quality management model and service solves these problems.
How do we help payers and employers?
Duplication of tests, avoidable emergency room visits for primary care issues, lack of care coordination, and lack of medication reconciliation are costly for payers and employers. We can reduce costs using our medical home model and by getting physicians involved in evidence-based care, quality improvement program and assuring that all of the doctors are on the same page for patient.
