Medical Power of Attorney

The Medical Power of Attorney is an important document that you will have available on-line. It designates the person that you want making decisions for you should you not be able to do so.  If you do not have one, you can use this one.  

If you have one and are a YDP member, we will scan it in for you so it is available for you. If you do not have one, you can use the one attached here. 

Living Will

A Living Will is an important document. It defines your wishes for the types of treatment you would want should a catastrophic event or major terminal illness arise. If you have one, we will upload it for you if you are a YDP member. If you do not have one, you can use the one attached here.
