Currently in today’s healthcare system, there is not a point of accountability for patients.  Everyone expects the primary care doctor or the insurance company to somehow be that for them.  Unfortunately, the financial incentives are not aligned for this.  The technology to support the doctor has not been in place.  Now the incentives and technology are.

As a Your Doctor Program Credentialed Physician you are practicing at the highest standards of practice in the country.   Continually meeting these standards is not easy but the financial and professional rewards for you, your staff, and your patients are unparalleled.  

The YDP Program provides the primary care physician additional payment for doing what is necessary to care for patients effectively to meet the YDP standard of practice.  For example, currently the physician is not paid for:

• Being on call
• Coordinating care among a doctor network
• Using telemedicine, e-mail, e-prescribing and telephone to improve efficiency of communication among patients and specialists
• Incorporating prevention and wellness programs into patient care
• Incorporating disease management in collaboration with specialists telemedically

Now with the advent of the outcomes movement to reduce medical errors, there is a growing trend to collect outcomes data on patient care.   Unfortunately, there is not a payment mechanism to pay for this.   Additionally, the processes are cumbersome and not physician driven or patient centered.   Using the YDP methodology for outcomes collection assures:  

• Minimal time for the physician and staff
• Integrates with your existing practice management system / electronic system
• Puts the physician in the driver's seat for outcomes management
• Assures that the patient understands their plan of care and role in their outcomes
• Provides on-line summaries of your referral's recommendations
• Incorporates personalized continuing medical education for your practice

Four Step Process

Step One: We initiate the YDP credentialing process by conducting an initial site assessment of your practice (technology, quality improvement processes, financial opportunities for improving management).

Step Two: We integrate the technology into your existing systems.

Step Three: We complete the YDP credentialing process by documenting your baseline treatment preferences (CME credit given), training your office staff on how to incorporate outcomes improvement and how to work with patients on their roles and responsibilities for their healthcare.  

Step Four: We publicly announce that your practice is meeting the highest standards of practice in the country and market your practice.
