Why is our service important? Getting physicians involved in evidence-based care, quality improvement and assuring that all of the doctors are on the same page for patient care is a challenge. Our quality management model and service solves these problems.
What is our service? The Your Doctor Program, L.P. has developed a one day implementation program for a hospital to work with physician staff to implement evidence based practice and processes to improve communication both within the hospital and with the referring physicians.
To accomplish this we use a tool called the Quality Health Record (QHR), a communication tool for patients and doctors. Key elements of the QHR are:
- Protocol customization by physician based on national guidelines
- On-demand “curbside consultation” to specialists for disease management (pharmacy, radiology, pathology, psychiatry, cardiology, endocrinology and others based on need)
- Live interactive telemedicine to specialists
- Specific patient goals based on both national guidelines and physician preferences
- Secure messaging among physicians for outcomes collection and future peer review
- Patient risk factor data recorded at the time of care delivery by disease state for adjusting patient outcomes
- Reconciliation of treatment plans and medications among treating physicians
- Outcomes follow-up information from patients on quality of care and their understanding of treatment plan
How do we overcome the physician barriers to engagement in quality improvement? We have established a practice-based continuing education program focused on quality improvement using Pay for Performance and Peer Review.
