Patient Enrollment

Keep the following ready for enrollment:

1. Your email ID. If you don't have one, sign up here to get one.

2. Payment information (Paypal or a Coupon code you received from YourDoctorProgram )

Below are the steps for enrolling as a YDP patient

Step 1 : You need to digitally sign an AGREEMENT with YourDoctorProgram.com in the next page.

Step 2 : Pay for the service with a Paypal account or a Coupon code received from YourDoctorProgram.com

Step 3 : Enter you Demographic information and choose your Medical Home physician to manage your care

Step 4 : You will receive a confirmation email to your mail box with a link to an online survey you need to complete before scheduling your first visit with YDP  physician

Step 5 : You will receive a phone call within 24hrs from the time of enrollment from YDP staff to get you started
