Your Doctor Program, L.P. Services for Patients

We provide the answer to the questions, "who is a good doctor" and "how do you find one?".  Once you are enrolled into a YDP Certified Medical Home, we provide a streamlined method for managing your health information to assure that care is coordinated and your doctors are on the same page.  We provide 24 / 7 access to a doctor that has your medical information.  We increase access to physicians through email and telemedicine.

What is our service?

Medical Home Service:   The Your Doctor Program Medical Home Service (YDP-MHS) has established a network of physicians that meets our quality standards and has incorporated telemedicine into their practice.   This will provide the following services to you or your loved one:    

• 24 / 7 access to a physician network that has your information and knows your care plan.
• A reconciled care plan among your doctors so that they are on the same page. 
• Secure messaging and email with your doctor.  
• Live interactive telemedicine to specialists.
• Feedback from you on how we and your doctors are doing. 
• A location for secure storage of your medical records. 

What does the service cost?   There is an initiation fee and a monthly service fee.  The initiation fee is $100 and the monthly service fee is $25-100 depending on your health status.   Your regular doctor visits, labs, imaging, and hospitalizations are covered by your insurance.   This service is approved for flexible spending accounts.   If your insurance pays for the service at the time of enrollment, you will be given a coupon code to use for enrolling into the service.
