Service Description:

  • Initiation: We conduct a comprehensive assessment, enter your information into our secure online record system for core information management, conduct laboratory assessment if indicated, and develop a personal health roadmap.
  • Ongoing Service: 365 days a year, 24 hours a day, 7 days a week, you have access to a YDP Certified doctor that has your information and will help you manage your health whether you are traveling, get sick at home, or have to be admitted to the hospital.   

Levels of Service

There are three levels of service depending on the patient complexity:
  • Level One: Wellness and Prevention
  • Level Two: Chronic Disease Management  
  • Level Three: Intensive Care Management

 

Level One: Wellness and Prevention

Why is the Wellness and Prevention Program Important?

Prevention is well known to improve quality of life and to save money, yours and the employer.   The prevention program is designed to identify risk areas and to build a personal wellness roadmap.  Ongoing support is given to help the individual meet their personal goals.  Confidential reports are given to the individual on how they compare to their peers in meeting their goals.  We focus on smoking cessation, weight reduction, and screening for chronic illness.

What patients would benefit from the service?

  • All patients.  

What are the fees?

  • Annual Fee: $250
  • Monthly Fee: $25

Level Two: Chronic Disease Management  

Why is this program important?

  • Chronic diseases such as diabetes, hypertension, heart disease, asthma are very important to monitor carefully.  

What patients benefit from the service?

  • Patients with several chronic conditions.  

What are the fees?  

  • Initiation Fee: $250
  • Monthly Fee: $50

Level Three: Intensive Care Management

Why is the program important? 

  • This program is designed for individuals who have complicated patient problems and that are either hospitalized, see multiple physicians, or are homebound.
  • This program assures the coordination of care for a patient to keep all of the doctors on the same page.  We also assure that you and your family understand the plan of care. 

What patients would benefit from the service?

  • You live in another city from your parents or other loved one.   They rely on you to help with their care system.   You'd like to have assurance that their care plan is coordinated and that they understand their plan.  We make sure of it.  We will contact the appropriate doctors and provide an understandable analysis of the care plan. We will assure that outcomes of care are collected and managed effectively. 
  • You have a family member that is suddenly hospitalized.  You and the rest of the family have no idea what is going on with their care and don't know how to get answers.   We do.  We will contact the doctors and assure not only communication with you but communication among the doctors.

What are the fees?

  • Initiation Fee: $250
  • Monthly Fee: $100

Telemedicine Services

Telemedicine uses advanced telecommunications to provide care at a distance. We currently have Telepsychiatry available 24 / 7. Other specialties are available on an as needed basis. We are adding specialties during 2008 for 24 / 7 capability.

  • Ever want an economical way to get a second opinion for yourself or a loved one?  Ever have questions about the level of care being provided?

What patients would benefit from this?

  • Patients who have multiple doctors or who have the need for second opinions. 

How does it work?

  • Step One: Email us with a description of your need.
  • Step Two: We contact you to schedule an appointment.
  • Step Three: We conduct an intake process.
  • Step Four: You have a scheduled visit with the physician or we can have a physician to physician consultation to determine if the care plan is appropriate. 

We can also place a live interactive telemedicine kiosk on site for large employers to make access to specialty care.

Consulting Services

The Your Doctor Program model of quality management can be useful to physician groups to meet Pay for Performance Programs, and Hospitals and Payers for their quality programs and peer review programs. The model can also be helpful to assist physicians and hospitals with clinical integration.   

We have developed a very efficient and cost-effective method for assisting groups to adopt an integrated outpatient/inpatient quality assurance program.   We can also assist your organization to develop telemedicine programs domestically or internationally.
