How is Your Doctor Program different?
Here are a couple of Common Scenarios
Care Access for Common, Predictable Situations
- Current Model: You get a cold or infection. You know what you need, but for the doctor to get paid, you have to schedule an appointment or find an emergency center, particularly if you’re traveling. You’re seen, but without follow up or coordination of this scenario within your overall health.
- YDP Model: You call or e-mail your doctor and discuss the problem. A cost-effective prescription is called into the pharmacy of your choice. Additionally, the information is recorded in your personal care record and you receive a follow up assessment on the treatment’s effectiveness.

Ongoing Illness Prevention- Current model: Your doctor tells you to lose weight, stop smoking, and exercise. You agree but without a plan on how to get there.
- YDP model: Your doctor and you agree on an annual healthcare goal and plan. You have a schedule of events, on-line and other resources along with reminders and follow-up visits scheduled to assure progress.

Healthcare Coordination
- Current model: You have a number of specialists. You sometimes wonder if they ever talk about you as a whole person because you keep getting duplication of tests and services. Or you get admitted to the hospital and you have no one who is your advocate to assure coordination.
- YDP model: Your personal care physician coordinates care in a patent-pending process using the Your Doctor Program Patient Summary. It keeps doctors on the same page for your care. If you need to be admitted to the hospital, your doctor works with the attending physician to assure that a) you know what is going on and b) outcomes are collected and medical errors are reduced.

Understanding Your Condition(s) and Treatment (s)
- Current model: You leave the doctor office with a set of medications but don’t know which ones are for what. And you have a lot of questions.
- YDP model: You and your doctor agree on specific goals for each problem and review your other problems that are managed by other doctors. The YDP certified outcomes manager for the practice provides you with a YDP Encounter Summary to make sure you understand the problem, treatment, and goals. You e-mail your questions to your doctor.

Healthcare Outcomes
- Current model: Currently, there are 98,000 preventable deaths from medical errors every year. That is one jumbo jet of people a day. There is not a patient-centric model for monitoring care to prevent errors.
- YDP model: You have a physician advocate and monitoring system that collects outcomes on all of your healthcare encounters which are then peer-reviewed by other doctors.

End of Life Decisions
- Current model: You know you don’t want to end up like Terry Schiavo. You have thought about having a living will. Maybe you get one and it is in your home someplace or you just don’t get around to it and something bad happens. Your family is left with the difficult and painful decisions.
- YDP model: A YDP standard of practice is to help patient and their family develop an emergency roadmap that includes a Living Will, a Power of Attorney, and an emergency contingency and notification plan. These documents are available on-line to you and your family.
Your Doctor Program Certified Practice
A Your Doctor Program certified primary care physician is the patient’s gateway to care and the accountability manager for the patient in the healthcare system.
The YDP Program provides the primary care physician additional payment for doing what is necessary to care for patients effectively to meet the YDP standard of practice. For example, currently the physician is not paid for:
- Being on call
- Coordinating care among a doctor network
- Using telemedicine, e-mail, phone to improve efficiency of communication among patients and specialists
- Incorporating prevention and wellness programs for patients
- Incorporating disease management incorporate disease management in collaboration with specialists available telemedically to you on demand
Now with the advent of the outcomes movement to reduce medical errors, there is a growing trend to collect outcomes data on patient care. Unfortunately, there is not a payment mechanism to pay for this and the processes are cumbersome and not physician driven or patient centered. Using the YDP methodology for outcomes collection assures:
- Minimal time for the physician and staff
- Integrates with your existing practice management system or electronic record system
- Puts the physician in the driver’s seat for outcomes management
- Assures that the patient understands their plan of care and their role in their outcomes
- Provides on-line summaries of your referral’s recommendations
